Sustaining Long-Term Stability

Support doesn’t stop once someone is housed — we continue working alongside individuals to help them maintain their accommodation and build a stable future.

Housing & Accommodation

Structured, person-led support

Our housing support is built around each individual — recognising that everyone’s journey is different.

Through our Accommodation Project, individuals are supported to:

  • Set personal goals and identify areas they want to develop
  • Build confidence in managing their home and daily life
  • Access healthcare, employment, and support services
  • Address challenges such as finances, mental health, or substance use

Support is delivered through regular one-to-one sessions, shaped around what matters most to each individual.

We focus on building stability, confidence, and the skills needed to move towards independent living.

Intensive Transition Team (ITT)

Our Intensive Transition Team supports individuals with more complex needs to move into and sustain accommodation.

Funded by Worcester City Council, this service provides targeted, intensive support to help individuals transition from homelessness into housing, engage with services and stabilise their situation, and reduce the risk of repeat homelessness. This work is key to supporting those who may otherwise struggle to maintain accommodation without additional support.

Outreach, Day Centre & Hub Support

Our Outreach teams, Day Centres and hubs play a vital role in supporting people into accommodation.

They work to identify individuals who may benefit from housing, support referrals into appropriate pathways, and provide ongoing engagement throughout the process. By helping individuals prepare for and sustain accommodation, this approach ensures people are supported before, during and after accessing housing.

“Housing created the foundation for a fresh start.”

After experiencing domestic abuse, exploitation and homelessness, Sarah returned to Worcestershire with nowhere safe to stay.

Over a number of months, Maggs worked alongside Sarah and partner organisations to support her through periods of rough sleeping, temporary accommodation, poor mental health and substance use challenges.

Staff provided regular support, advocacy and practical help — including attending appointments, arranging essentials for accommodation, supporting safeguarding processes, and helping her engage with recovery services.

As trust developed, Sarah began focusing on longer-term goals for her future, including rebuilding stability and reconnecting with her children.

With support from housing partners, she was eventually able to move into permanent accommodation before later accessing residential rehabilitation support to continue her recovery journey.

Her journey has not been straightforward, but with consistent support, stability and encouragement, Sarah has continued taking positive steps forward.
