Meeting people where they are
We understand that traditional healthcare systems can be difficult to access, particularly for individuals experiencing homelessness or instability.
Our approach is:
- Flexible – support is provided in day centres, outreach settings, and community locations
- Relationship-based – building trust over time
- Person-centred – led by individual needs and priorities
We work alongside health professionals and partner organisations to ensure people receive the right support, at the right time.
“Recovery became possible once the right support was in place.”
After experiencing rough sleeping and unstable accommodation, Daniel was struggling with both his mental health and substance use.
Working alongside health partners and support services, Maggs helped Daniel access safer accommodation, reconnect with healthcare, and begin engaging with support around his wellbeing and recovery.
This included support from community mental health services, reconnecting with substance misuse support, attending appointments, and accessing practical help to settle into his new home.
Alongside improving his housing situation, Daniel was also supported to access a bank account, furniture and household essentials — helping him begin rebuilding stability and independence.
Today, he continues to engage positively with support and is making ongoing progress in his recovery journey.

Harm Reduction
Our Harm Reduction service provides non-judgemental, practical support to individuals experiencing substance use challenges. We offer needle exchange and safer use advice, Naloxone distribution and overdose prevention, and support accessing treatment and recovery services.
Alongside this, we provide ongoing engagement to help improve health outcomes, supporting individuals to reduce harm and stay safe wherever they are on their journey.

Day Centre Health Support
Our day centres provide a trusted and accessible environment where individuals can engage with health support more easily. We deliver a coordinated in-reach model, working with visiting professionals and partner organisations to bring healthcare into spaces people already feel comfortable accessing. This includes regular nursing clinics, mental and sexual health services, substance misuse support (including Cranstoun), BBV testing through the Hep C Trust, and access to specialist services such as opticians and cancer care coordination.
Some services are delivered through regular weekly sessions, while others attend flexibly based on need. Our team supports individuals to engage with these services, attend appointments, and continue their care — helping to improve access, build trust, and support long-term health outcomes.
Outreach Health Support
Our outreach teams work within the community to engage individuals who may not otherwise access services. We provide health-related support in community settings, connect individuals to appropriate services, and offer ongoing engagement and follow-up, particularly for those with complex or multiple needs.
This includes regular outreach sessions across Malvern, Evesham and other community locations, ensuring support reaches people wherever they are and helping to reduce barriers to accessing care.
